Individual Will with Testamentary Trust. Includesthe individual will with testamentary trust, and the following ancillary documents: decleration of guardianship, directive to physicians, durable power of attorney, gift by a living donor, medical power o attorney and appointment of guardian. Limitation--- must be redeemed by February 1, 2017. This is only for a single person.
